This application is for former students returning only. Please complete this application thoroughly and thoughtfully. When completed, you must attach your Personal Statement with any documentation. Applications WILL NOT be accepted in the mail via United States Postal Service. Students must either hand deliver their personal statement and application materials or email them to their advisor prior to the reinstatement interview.
